We are looking for a Full-time Community worker who can be able to support community/people dealing with daily living problems including physical, emotional and personal and refer them to agencies in case they require any additional support.
Requirement:
- Meet with individuals and families to understand their circumstances, goals, and support needs.
- Create tailored support plans that reflect personal strengths and outline clear steps and outcomes.
- Connect clients with essential services such as housing, healthcare, mental health support, financial counselling, or advocacy services.
- Coach practical life skills like communication, self-advocacy, and independent living to help clients live with dignity and confidence.
- Check in regularly with clients, monitor their progress, and adjust support plans as needed.
- Organise and run small group activities or community workshops that reduce isolation and build a sense of connection.
- Maintain accurate and confidential case notes, assessments, and progress reports in line with privacy and organisational standards.
- Advocate for the rights, dignity, and safety of all clients, ensuring services are respectful, inclusive, and culturally aware.
- Prepare basic funding proposals or program reports for government bodies or partner organisations.
- Contribute to local awareness efforts by participating in community outreach or information sessions.
- Deliver short-term support and referral pathways for priority groups such as young people, carers, people with disability, and CALD communities.
Skills and Experience
To succeed in this role, the ideal candidate will bring a genuine passion for helping others, along with the practical experience and qualifications needed to support people living with disability, ageing-related challenges, or complex personal needs. We are looking for someone who can confidently work both one-on-one with individuals and collaboratively within a wider care team.
The ideal candidate will have:
- At least 2 years of hands-on experience working in community services, aged care, NDIS support, or in a residential or nursing home environment.
- A relevant qualification, such as a Diploma in Community Services, or a Certificate III or IV in Individual Support, Disability, Aged Care, or a related field.
- Strong communication and interpersonal skills, with the ability to build trust, listen actively, and adapt communication styles to suit people from diverse backgrounds.
- Patience, empathy, and resilience, especially when supporting individuals facing complex emotional, physical, or behavioural challenges.
- A current Australian driver’s licence, as travel between client homes and service locations may be required.
- A valid National Police Check, Working with Children Check, NDIS Worker Screening Clearance, and up-to-date First Aid and CPR certifications.
- Ability to maintain accurate records, follow care plans, and work within privacy and safety guidelines.
- Flexibility and reliability, with a strong work ethic and commitment to person-centred care.
Experience working with people from culturally and linguistically diverse backgrounds or Aboriginal and Torres Strait Islander communities is also highly valued.
Availability & Locations
Multiple full-time positions are available across Victoria and South Australia, with vacancies in the following locations:
- Mildura, Swan Hill, Ballarat (VIC)
- Renmark (SA)
You will be assigned to one primary location based on your preference and service needs. Initial accommodation support will be provided to help you settle into the community.

Penalty Rates & Bonuses
This role attracts penalty rates for weekends, public holidays, and overtime in accordance with the relevant award. In addition, performance-based bonuses may be available based on service outcomes, client feedback, and team contribution.
